Among other things, this page includes a number of forms that may be useful to you including sheets to track and submit your hours of service (which will then be reconciled for recognition and awards). We certainly encourage you to use this feature so we know how many hours you are devoting to the organization and the animals!

Note: the following forms are made available as PDF Fill-in Forms (so you may type or check mark your responses in the open file by tabbing through the boxes, print the file and return via email or fax). You will not be able to save your form data locally.

 

Speaker/Presenter Request Form (PDF) (for schools and other groups wishing to arrange a presentation)

 

Special circumstances are listed below - please review prior to submitting your application.

Underage volunteers must be accompanied by a parent or guardian during all service hours. Minimum age of 15 for student volunteers - suggested activities for youth volunteers under age 15 can be found on our Kids' page.

Court Ordered Community Service workers: We do not process court-ordered CS workers via our regular application. If you have hours to be completed and wish to know whether you can support HSFC, please contact our main office at 703-385-7387 and advise your requirements. If we have work available that is suitable, the manager will direct you as appropriate. Thank you.

*** Students Aged 15 - 17: IF YOU ARE VOLUNTEERING BECAUSE YOU NEED COMMUNITY SERVICE HOURS FOR SCHOOL, WE MUST KNOW THIS IN ADVANCE. Thank you.

 

Application and Agreement (PDF) (for new volunteers age 18 and older) (Note: applications are reviewed and processed weekly so you should receive a reply within seven days of submitting your completed application and agreement).

NOTE: Submitting your application and agreement is the required first step on the path to volunteering. Please do not email just to say you would like to volunteer - your forms MUST be received for processing. Thank you.

Student Application and Agreement (PDF) (for new volunteers aged 15 to 18). Form must be signed by a parent/guardian AND must be accompanied by the P/G's signed Application and Agreement acknowledging participation. *** Minimum age for student volunteers is 15 for work at any of our facilities.
